I would like the legal entity (entity without legal autonomy) represented by me to be entered in the Unified Trade Register of BCCI on the grounds of Art. 14, para 5 and in relation to the services provided by BCCI pursuant to Art. 9 of its Articles of Association.

Enclosures:

1. Registration Card for the information system of BCCI with enclosed documents;
2. Notarized Power of Attorney from the legal representative of the applicant (if the Application Form and the Registration Card are filled in, signed and submitted with enclosed documents, by a person who is not the legal representative).
